All district Back-to-School materials are online. We believe this will make it easier for families and our community to find the materials you need. And, it will help save money and resources!

Step 1  Please read the following forms.

• Parent-Guardian/Student Rights and Responsibilities  The law says we must give you this information at the beginning of the year. 

• Standards for Student Behaviors  This will help you explain our district’s behavior standards and policies to your child

• Agreement for Acceptable Use of Technology  You and your child should understand his/her responsibilities for use of technology in school.

  You may fill out and return this form to tell us which agencies may/may not receive information about your child.

• Permission to Display Student Work
  You may fill out and return this form if you don’t want your child’s school work or photos displayed.

• Free and Reduced Cost Lunch Application  and Information on Lunch Pricing
  You may qualify for free or reduced-cost lunch. These forms can be confusing. Please ask your school site for help.

• MyNutriKids
  This is a new service that makes paying for school meals easier. 
• Low-cost Preschool
  Information about this community-based program.
• GATE Program
  Our district tests all 3rd graders ("universal testing") for GATE placement.  If you do not want your child to be tested, print and sign this form and send it to your child's school.  For more information about the GATE program, click HERE.

• Instrumental Music Program
  Information about band and orchestra lessons in our district.

• City of Davis Teen Program  The City of Davis provides many activities, including dances, for Davis teens during the school year.  Click this link for information about the programs, as well as the parent/guardian permission slip necessary for participation

• Student Accident Insurance
  Information about low-cost, short-term insurance, including for tackle football players and other sports. Insurance is required for school athletics. If you already have insurance, you don’t need to purchase more.

  Information about Medi-Cal health insurance for low-income families.

• Information about Pesticide Use and specific chemicals that could be used. Fill out and return the form if you want us to tell you each time pesticides are used.  Click here for the Master Pesticide List.

Step 4  Your child’s school site will provide you with additional forms to complete registration.
