Referral Form for Outside Evaluation
A "Referral Form for Outside Evaluation" must be picked up and filled out at the DJUSD District Office after an appointment with an Outside Evaluator(Psychologist) has been made but prior to the testing. The outside evaluator will then complete the form. Test results will NOT be accepted without this form.
