Referral Form for Outside Evaluation
A "Referral Form for Outside Evaluation" must be picked up and filled out at the DJUSD District Office, 526 B Street, after an appointment has been made with an outside evaluator, but prior to testing. The form is then taken to the testing appointment and completed by the Outside Evaluator. Test results will NOT be accepted if this form has not been completed in the prescribed manner.
