May 13 Update: Due to overwhelming demand, we are adding a drive-thru vaccine clinic for this Sunday at Harper from 10:00 a.m. - 4:00 p.m.

The Sunday clinic is open to any Yolo County student that is 12+ and their families. Students do not need to attend a DJUSD school.

Details:

  • Sunday, May 16 from 10:00 a.m. – 4:00 p.m.
  • At Harper Junior High (4000 E. Covell Blvd, Davis, CA 95618)
  • Administering 1,000 Pfizer doses
  • Drive-thru (it’s not in the flyer but we will also take those that walk up or bike up)
  • Sign up through MyTurn (https://myturn.ca.gov/), but we will also take those without an appointment
  • A minor does not need to have their parent/guardian present but they will need a completed consent form to be vaccinated
  • Here are the links to the minor consent form, which are also on the flyer
  • If you decide to attend the Sunday clinic instead of the Tuesday clinic, you do not need to cancel your Tuesday appointment

Original Story: A Pfizer COVID-19 Vaccine Clinic for students age 12 and over will be held at Harper Junior High (4000 East Covell Blvd. in Davis) in the gym on Tuesday, May 18, from 2:00 - 6:00 p.m. Please use this Google form to sign your student (12 years and older) up for a vaccine slot. Appointments are not required but completing this form will help inform us on the number of people arriving at once. Any additional unvaccinated family members may also be vaccinated at the same time.

Please be aware that the student must receive the second vaccine shot in addition to this first shot, which will be administered at another clinic held on June 8 from 2:00 - 6:00 p.m. Parents do not need to be present with the student at the vaccine clinic. Any student arriving unaccompanied (without a parent present) must submit the completed and signed COVID-19 Screening Questionnaire.
